Bridging Clinical Care and Community Recovery

The Integrated Recovery Pathway (IRP) is designed to strengthen the transition between clinical mental health care and long-term community recovery.

Too often individuals move between hospital, residential recovery and community services without clear continuity of support. The Integrated Recovery Pathway addresses this challenge by dissolving traditional “silos” between clinical services and community-based psychosocial support, ensuring that individuals are supported at every stage of their recovery journey.

By strengthening collaboration between clinical services and Sunflower House, the pathway provides a structured and supportive transition from hospital care to sustained community wellbeing.

A Unified Continuum of Care

The Integrated Recovery Pathway establishes a warm-handover framework connecting three essential pillars of the regional mental health system.

Acute Mental Health Care

For individuals experiencing the most intensive stage of mental illness, the pathway ensures that discharge planning begins early, with a clear focus on recovery beyond the hospital.

Rather than leaving hospital without structured follow-up, individuals are introduced to recovery supports that will assist them once they return to the community.

The Recovery Unit

As individuals transition from acute care into sub-acute or residential recovery settings, the pathway provides continuity of support.

During this stage individuals begin reconnecting with daily routines, social engagement and life skills, helping reduce the isolation that can often follow a hospital stay.


Community Mental Health & Sunflower House

Sunflower House provides the community-based psychosocial recovery environment that completes the pathway.

By introducing individuals to Sunflower House before they leave clinical care, participants experience a “soft landing” into community life where they can access:

  • Peer-led recovery support

  • Structured psychosocial programs

  • Social connection and community participation

  • A safe and welcoming recovery environment

This early connection helps build confidence and stability during the transition from clinical care into everyday life.

Reducing Hospital Readmissions

By strengthening community support immediately following discharge, the pathway helps reduce the “revolving door” cycle of repeated acute hospital admissions.

Integrating Lived Experience

Recovery is strengthened through peer-led support, recognising that clinical stability is only one part of the recovery journey.

Supporting Holistic Recovery

The pathway moves beyond symptom management to support individuals in rebuilding their lives through:

  • Social connection

  • Daily living skills

  • Confidence and independence

  • A renewed sense of purpose within the community


Why This Pathway Matters

Across Australia, mental health systems face a major challenge: the transition between clinical treatment and long-term community recovery.

While hospitals and clinical teams provide critical care during periods of acute illness, many individuals struggle when returning to everyday life without structured psychosocial support. This gap can lead to social isolation, loss of confidence, and in some cases a return to crisis care.

The Integrated Recovery Pathway helps address this challenge by strengthening collaboration between clinical services and community-based recovery support.

By connecting individuals with Sunflower House before they leave clinical care, the pathway provides continuity, stability and a supportive environment where recovery can continue beyond the hospital setting.

The Integrated Recovery Pathway represents a collaborative approach to mental health care across the Riverina.

By working alongside the Murrumbidgee Local Health District, the Murrumbidgee Primary Health Network and community mental health services, Sunflower House helps ensure that individuals leaving clinical care do not face recovery alone.

Through this initiative we are building more than a program — we are strengthening a community safety net, ensuring that every individual has a clear, supported and dignified pathway back to the life they choose to lead.
